A Clinical Case Manager in Cypress, CA is available through Adecco Medical and Science. You will need a master's degree or higher in a health related field. You will also need a BSN and at least three years' behavioral health clinical practice. You will be responsible for managing the members who are categorized under Intensive Care Management (ICM). The members are often high risk, high utilizing, at significant clinical risk or under-utilizing services.
Managing, coordinating care, tracking and reporting of all assigned Members
Collaboration with Community Support Programs (CSP)
In charge of regions: Massachusetts and New York (not Rhode Island) for assigned member covering all levels of care
Work as a unit to organize a supportive environment and provide clear communication
Update and sustain accurate information in the Beacon clinical documentation systems
If necessary will assist with collecting information, annual analysis and other reporting
Work with provider and county agencies regarding members in care management through all means is needed
Collaborate with Primary Care Physician (PCP), Behavioral Health Professionals (BHP), and other members of the health care team, including health plan medical care managers, Pharmacy, and others to arrange and coordinate services for the member and optimize the member's ability to engage in the appropriate plan of care
Manage and oversee plan of care for each assigned member, adhering to timelines and including assessment of health needs, individualized care management plans, implementation, monitoring and evaluation of care outcomes
Assist with NCQA and URAC initiatives
Communicate with and collaborate with members
Treatment Record Reviews
Provide Utilization Review assistance to non-ICM members if needed
Attend all require meetings
Master's degree or higher in a health related field and licensure as a behavioral health professional.
BSN, licensure as a professional and 3 years' behavioral health clinical practice
Licensed behavioral health clinician/nurse (LCSW, LMFT, LPCC, RN, Ph.D. or PsyD.) Certification as a Care Manager is preferred.
Demonstrated work experience to influence and negotiate to effectively manage patient care and health care outcomes.
Demonstrated work experience meeting strict deadlines and established cycle times through effective prioritization and follow-up.
Educated in current principles and procedures of behavior health care. Knowledge of managed care and state specific expertise preferred.
Strong interpersonal skills and good written and verbal communication skills.
Advanced computer skills required, including working knowledge of MS Office: Word, Excel, and PowerPoint
