Job Description

o Provide Pharmacy review of medication orders prior to administration when not an urgent/emergent indication and as indicated.
o Provide double-checks with nursing for pediatric medications that must be prepared when available.
o Provide double-checks with nursing for high alert medications (i.e. alteplase, heparin) when available.
o Work collaboratively with other clinicians and health-care providers to implement and maintain innovative disease management programs and clinical pharmacy services.
o Participate in the development of medication use management programs within Henrico Doctors' Hospital including core measures, clinical guidelines, critical pathways, disease management, and drug use programs.
o Provides medication history assessment for risk-stratified patients
o Focus on cost avoidance and cost savings due to medication use in the Emergency Department.
o Actively participates in ED codes (i.e. Code Purple, Code Stroke, Code Blue, Code 99, Code Sepsis, Code STEMI) when available, assisting in running the medication portion of the code carts, providing medication related information and recommendations, dosing and preparing of emergency medications for immediate administration.
o Facilitate proper information transfer with regard to medication use for patients converted to Inpatient admission status.
o Participates collegially in the development of new programs, services and practices, in the education activities of the department and the management and administration of the department.
o Keeps informed of all local, state and federal laws covering the storage, handling and dispensing of drugs.
o Keeps informed of the action, side effects and proper usage of new drugs as they are made commercially available as well as of all investigational drugs being studied at this institution.
o Participates in providing didactic and experiential training in Clinical Pharmacy for doctoral (PharmD) students and pharmacy residents.
o Actively participates in the provision of training and mentorship within peer group and cross campus pharmacists in Emergency Medicine service line including pharmaceutical care and disease state management.
o Participates as active committee member as the pharmacy representative for all committees within service line and additional as assigned.
o Actively participates as a member of the local, state, and national pharmacy societies.
o Creates national visibility by presenting clinical information/programs such as poster presentations or managed case studies or publishing in nationally recognized journals.
o Takes a lead role in working within clinical initiatives, improving patient safety and medication use.
o Facilitates or assists in the medication utilization evaluation process for service line.
o Documents all clinical activities and interventions accurately and completely.
o Supports and participates in the goals of the organization's quality improvement, risk management, and patient safety programs to include but not limited to National Patient Safety Goals, Core Measures, and regulatory compliance and HCAHPS medication-related initiatives.
o Performs drug class reviews and prepares drug monographs for committee submission and review.
o Participates in all aspects of formulary management.
o Performs other related duties as required.
o Complete all approved pharmacy consults for ED and ED Hold patients.
o Provide medication discharge counseling for all patients discharged on anticoagulants or high alert medications.
o Competent to fulfill Clinical Pharmacist I job responsibilities when needed.
Qualifications:
REQUIREMENTS :
Licensure by the Virginia State Board of Pharmacy by examination or reciprocity and meets the following criteria:
• Doctorate in Pharmacy (PharmD) or equivalent experience
• Successful completion of PGY-2 (Emergency Medicine or Critical Care) or at least 3 years of practice in Emergency Medicine
• BLS, ACLS, and PALS or obtain within 3 months of hire
• Board Certification in Pharmacotherapy Specialty or Board Certification in Critical Care Pharmacotherapy or obtain within one year of hire (based on time period of when testing is offered)
• Well versed in medication management and pharmacology in the following areas:
o Airway management, RSI/Post - RSI sedation
