BAYADA Home Health Care is pleased to announce an open position in the New England Region.  The Director of Accountable Care is a key member of the regional leadership team responsible for ensuring accountability with strategic partners and high level client relationships.

The Director of Accountable Care will:


Manage internal relationships at the office, divisional and regional levels:

  • Serve as subject matter expert for strategic relationships within designated service region, working with the Regional Director, Division Director and Director of Clinical Operations. Create accountability for strategic relationships, including those that span multiple BAYADA service offices or have the potential to produce a large volume of referrals for the organization. 
  • Work closely with the Regional Director and the Division Director to ensure internal service office teams meet project deliverables and timelines.  Facilitate regular communication among key relationship owners, including Regional, Division and Office Directors as well as Directors of Area Sales and Directors of Clinical Operations. 
  • Maintain up to date knowledge regarding changing healthcare payment & delivery models. Provide education and support to leaders throughout BAYADA in regards to these changing models.
  • Facilitate leadership and accountability of the service office Director in maintaining relationships with health systems or other providers. 
  • Participate in leading large-scale organizational change including initiatives in payment, technology, and workforce management related to strategic relationships.
  • Support the internal payer strategy group in negotiating various payment methodologies for strategic relationships.  Serve as subject matter expert for developing innovative provider payment or delivery arrangements.


Support operations and business development teams with external relationship management:

  • Provide ongoing communication regarding partner relationship status and expectations to Regional and Division Director within service area. Facilitate and support care innovations requested by external partners and work with internal resources to operationalize.
  • Coordinate response to Requests for Proposals/Information (RFP, RFI). Collaborate with Regional, Division, and Office leaders, as well as the Director of Clinical Practice, to complete and submit on-time.
  • Work collaboratively with the Sales and Marketing team to provide subject matter expertise to prospective partners.
  • Participate in Regional growth planning and budget preparation to project potential admissions and activity costs when requested.


Perform in-depth data analysis to support clinical service delivery and integration:

  • In collaboration with the Regional, Division and Office Directors, provide consultation to internal and external partners on metrics and reporting capabilities.  Develop, implement and monitor performance metrics to ensure programs support coordination of care. 
  • Prepare and present best practice recommendations in the form of written and verbal reports and presentations to internal and external team leaders.
  • Maintain database for regional relationships; update Director of Clinical Practice with all new relationships and any changes with existing relationships quarterly and as requested.

Performance Outcomes:

Responsible (in collaboration with the Division Director) for the following Pillar-based BAYADA Key Performance Indicators (KPIs) within the Division:

  • Growth: Increase client admissions
  • Quality: Improve publicly reported outcomes
  • People: Increase retention rate for office staff
  • Finance: Increase/maintain episodic gross margin
  • Service: Increase client satisfaction scores

Qualifications for the Director of Accountable Care:

  • Exemplifies characteristics of The BAYADA Way; Compassion, Excellence and Reliability
  • Holds a current license in good standing as a Registered Nurse or Therapist in the state of practice
  • Four (4) year college degree
  • Three (3) to five (5) years of Home Health Care Clinical Leadership
  • Completed Continuing Education Requirements (CEUs) as required by regulation
  • Demonstrated expertise in high quality clinical service delivery and multidisciplinary team management
  • Knowledge of health care environment as it relates to acute and post-acute delivery systems
  • Experience working with new payment models
  • Ability to commit to on-going travel throughout the region
