Responsible for strategy and implementation of the Care Delivery Transformation programs.
Primary duties may include, but are not limited to:
Serves as lead for management and execution of transformation programs, including national and local strategy.
Works with the Program Development and Training team to inform development and implementation of learning collaboratives, webinars, and toolkit enhancement.
Develops metrics and goals that drive field staff documentation, interventions and execution to support organizational and LOB goals
Develops documentation to support internal and external presentations as well as tracking and monitoring the value and success of program implementation.
Collaborates with Care Delivery Transformation Directors and Business Change Directors related to program development and internal/external communication.
Accountable for developing and maintaining Internal LOB relationships as well as local market relationships.
Hires, trains, coaches, counsels and evaluates performance of direct reports.
Requires a BA/BS; 5 years of experience in external client facing program management, primary and specialty care transformation, quality improvement, and experience in a primary/ambulatory care setting; or any combination of education and experience, which would provide an equivalent background. Medicare programs experience preferred. RN and/or clinical background preferred. Transformation Program Development and Implementation Required. MBA preferred. Project management experience preferred. Travel required.
