The Social Worker provides assistance to patients, families and significant others to enable them to deal with the psychosocial issues that impact mental illness and to maximize benefits from health care services. Position includes psychosocial assessment, treatment planning as a member of multi-disciplinary treatment team, crisis intervention, discharge planning, referrals to appropriate community resources, evaluation of core measures (HBIPS) and networking with community services providers.Qualifications: Position Qualifications
- Bachelor Degree in Social Work required, Master of Social Work preferred
- FL LCSW required
- Current BLS (American Heart Association) Healthcare Provider
- 1-3 years behavioral health experience preferred
- Previous social work experience in an acute care setting preferred.
- Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
- Must be able to work in a fast-paced environment.
