Overview:

BAYADA Home Health Care is currently seeking an experienced Licensed Practical Nurse, LPN, for per diem hours performing home health visits for our Bennington, VT office. This office services adult and geriatric clients on a per visit basis in territories throughout Bennington County. Prior home care experience is strongly preferred, but not required.

As a home care nurse, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities:

Responsibilities for an LPN include:

 

  • Adheres to the established client care plan and the Physician’s Plan of Treatment.
  • Performs assigned duties, including administration of medications, treatments, and procedures as indicated, monitoring the client’s condition, etc.
  • Reports any changes in the client’s condition to the physician; executes and properly documents any new doctor’s orders.
  • Makes ongoing additions to the client’s care plan, as needed.
  • Understands and follows safety and infection control policies and practices.
  • Demonstrates sound judgment.
Qualifications:

Qualifications for an LPN include:

 

  • A current license as a Licensed Practical Nurse in Vermont. 
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
  • Prior home care experience strongly preferred but not required.
  • Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
  • Ability to travel to cases as assigned.
