Accountable for ensuring that documents and images are uploaded within determined time frames to support the complete medical record. 2. Abstract or enter/edit pre-defined data into electronic data system. 3. Quality reviews each batch, verifying correct account number, patient identifier, and date and document type. 4. Quality review abstracted data, ensuring the accuracy of information. 5. Analyze documents for relevancy and ensure correct patient information is in the appropriate encounter. 6. Makes sure that there are no errors of scanned or abstracted data, checking for completeness of the chart. 7. Prints batch header sheets used to identify the patient. 8. Prepares document for scanning. 9. Indexes by accurately selecting document header sheet for documents that need to scan. 10. Operates high speed and desk top scanners. 11. Addresses all batches with errors such as but not limited to unknown medical record number, account number, or document type. 12. Quality checking to ensure that each scanned document is of best possible quality, rescanning as appropriate. 13. Responsible for understanding the back-scanning criteria, and what information will be scanned into the historical account. 14. Performs concurrent scanning within determined time frames. 15. Rounds with facility collecting items to be processed. 16. Accurately indexes all non-patient care documents to electronic cabinets. 17. Locate documents for appointed staff as needed. 18. Imports digital protected health information in electronic medical records. 19. Answer phones, takes accurate messages, looks for papers as requested and practices the customer service standards. 20. Performs other duties as assigned.
