Current Need

The goals of the VCCN (Vantage Oncology Cancer Network) Clinical Operations Director are to facilitate patient-centered oncology care for VCCN patients at all affiliated practices in the national VCCN network, while maximizing efficient and cost-effective use of health resources through the use of practice transformation, clinical pathways, care management tools and oncology expertise. The VCCN Clinical Operations Director will collaborate with the practices to monitor VCCN patient care plans, where necessary acting as a liaison between the patients and their families, the multidisciplinary care team, and the payer.


Position Description

Drives generation of the VCCN clinical model and the development of the practice transformation platform to be deployed at all VCCN participating practices, using one's own clinical experience and Vantage intellectual property.

Leads practice transformation implementations with all assigned practices and oversees ongoing optimization of the program

Supervises the consistent provision of coordinated care within patient populations, with effective utilization of available resources, in all assigned practices.

Serves as resource in care coordination to the multidisciplinary healthcare team, including mentoring and training team members on care management in assigned practices

Reviews patient care plans and provides feedback to practices on possible improvements, especially for high risk patients, through regular care planning meetings and other avenues of communication.

May establish a plan of care with patients, providers, and payers that identifies and continuously reassesses cost-efficient, appropriate levels of quality care

Identifies opportunities for health optimization and prevention of complications in oncologic patient populations

Supports the training and implementation of VCCN tools in assigned practices



 

Qualifications

Minimum Requirements
5+ years experience in project management, including 2+ years managerial experience.

Critical Skills

Bachelor Degree in Nursing

10 years experience within population health and patient centered medical home

Strong professional level of knowledge and comprehensive clinical assessment skills in the adult oncology population and chronic disease management required.

Must have current state licensure and current CPR certification.

Experience managing teams of people, including clinical staff

Additional Knowledge & Skills
Masters degree in Nursing preferred

Oncology Nursing Society certification (OCN) is strongly preferred.

Prior Case Management, Process Management, Clinical Quality Oversight or Clinical Program Management experience a plus

Education
4-year degree or related field or equivalent experience
