_Nurse Disease Management I - Telecommute_
A current unrestricted CA, NV, and/or WA RN license is required (You must have one of these currently to be considered, will have to obtain the others once employed)
Responsible for participating in delivery of patient education and disease management interventions and for performing health coaching for members, across multiple lines, for health improvement/management programs for chronic diseases.
Primary duties may include, but are not limited to:
Conducts behavioral or clinical assessments to identify individual member knowledge, skills and behavioral needs. Identifies and/or coordinates specific health coaching plan needs to address objectives and goals identified during assessments.
Interfaces with provider and other health professionals to coordinate health coaching plan for the member.
Implements and/or coordinates coaching and/or care plans by educating members regarding clinical needs and facilitating referrals to health professionals for behavioral health needs.
Uses motivational interviewing to facilitate health behavior change.
Monitors and evaluates effectiveness of interventions and/or health coaching plans and modifies as needed.
Directs members to facilities, community agencies and appropriate provider/network.
Refers member to catastrophic case management.
Responsible for outreach, enrollment and management of Disease Management members.
Provides education and coordination to address gaps in care for 11 managed conditions to ensure cost effective and efficient utilization of health services.
Conducts assessment, planning, implementation, coordination, monitoring and evaluation of members.
_Specifics of role_
Work from home position anywhere within the US.
Full time position (Monday - Friday 40 hour work week)
A current unrestricted CA, NV, and/or WA RN license is required (You must have one of these currently to be considered, will have to obtain the others once employed)
This position will require you to obtain additional licenses , you must be willing to do this with assistance from the department
_Hours of Position (You must be able to work your shift in your time zone, no flexibility)_
Monday - Friday 8 AM to 5 PM PST
Monday - Friday 9AM to 6 PM MST
Monday - Friday 10 AM to 7 PM CST
Monday - Friday 11 AM to 8 PM EST.

_Required: _
AS/BS in nursing
3 years of condition specific clinical or home health/discharge planning experience;
or any combination of education and experience, which would provide an equivalent background.
Experience working with electronic documentation and multiple databases.
Knowledge and experience working with Microsoft Office Products (Word, Excel, and Outlook)
3 years of clinical RN experience
Current unrestricted RN license in CA, NV, or WA, even if you do not reside there
Current unrestricted RN license in current state
_Preferred:_
Prior case management experience.
Prior telephonic case management experience
Manage care experience
CCM strongly preferred
Previous work from home experience
