Position Summary:As a Nurse Patient Care Specialist with CVS Health, you will work collaboratively with physicians, staff, and other health care professionals, to develop, implement, and evaluate integrated comprehensive plans of care for our home infusion patients. Provide telephonic education and counseling to patients and caregivers of patients, initiating, or transitioning to PAH or infused therapies that require expertise in high tech therapies. The Nurse Navigator must have excellent written and verbal customer service skills and advanced computer skills in order to interact with key stakeholders and patients. Be an expert in education of patients with PAH and other rare chronic diseases requiring infusion therapy in their home recovery care plan. Assist as a liaison with patients and their families to physicians, staff, and other health care professionals to coordinate patient care across all settings and ensure high quality outcomes. Ensure best practices and high quality standards of health care are maintained across the continuum. You will also provide consultative services to physicians and nurses related to the patient's plan of care and the clinical services provided by CVS Specialty in collaboration with the Specialty Sales team. Required Qualifications:* Registered Nurse with current license in Illinois with an ability to be licensed in all 50 states. * Minimum two years Home Health experience or experience with patients that have rare and/or chronic diseases. * Minimum one year patient care coordination or navigation experience. * Have experience with Microsoft Office, Excel, Outlook and Word to document and track patient care activity. * Must have and maintain current CPR certification.Preferred Qualifications:* Registered Nurse with experience in the home infusion therapy or acute care setting with a focus on the treatment of rare chronic disease states
* Experience working with PAH patients
