The IHA NP will be responsible for performing Health Risk Assessments and possibly transitional care for patients discharged from hospitals to prevent readmissions.
  • Conduct comprehensive in-home health risk assessment to identify all active and chronic disease conditions as well as determine all physical, mental and social needs present at the time of the visit.
  • Educate and counsel patient and family on any conditions identified during assessment and screening procedures.
  • Make recommendations and communicate findings to Primary Care Physicians through documentation of outcomes.
  • Performing follow up visits in transitional care
  • Current RN and NP licensure in state of practice
  • Current CPR certification
  • Must be enrolled in both Medicare & Medicaid
  • Minimum of 1 year of clinical experience as a Nurse Practitioner
  • HRA experience
