Physician, In Home Medical Care



BAYADA House Calls is currently seeking an experienced Physician (MD) for a per-diem position providing house calls to our patients in their homes. This is an exciting opportunity with an innovative and pioneering house call practice for a primary care provider who is looking to provide high quality care and make a difference in the lives of our home bound population.  You will have an opportunity to build a dedicated panel of patients that you will be able to build a personal, one on one relationship with. By utilizing a strong team approach combined with technology, we offer clinical services in many locations including private homes, senior residential facilities, group homes and skilled nursing facilities. Your visit becomes a personal experience with the focus on improving the quality of life of your clients. With a strong office team supporting your work, clinically and administratively, you will be allowed the opportunity to focus your time and energy on providing in depth quality care to your patients. Unlike a clinic, hospital or office based setting the time spent with your patients will range from 20-60 minutes depending on the visit, and the volume of patients you are managing and overseeing is significantly less. This personal take on primary care is ideal for a physician who is passionate about investing in their patients and their quality of life. The ideal candidate will have prior experience in house call medicine, home care, geriatrics, palliative care or hospice.

The Physician conducts in-home medical visits to senior and adult clients within a geographic area of responsibility, assuming all primary care oversight for a dedicated patient panel. Visits can include annual health assessments, post hospital or sub-acute discharge, or visiting clients with more complex medical conditions. In addition to the assessment process, the Physician is responsible for identifying gaps in care, ensuring optimal care coordination, and providing teaching and disease management to clients as needed.


Key Job Functions:

  • Performing in-home primary care visits to an assigned patient population (averaging 10-15 patient visits per day).
  • Contributing to the interdisciplinary care plan with the rest of the care team in a weekly IDG meeting setting
  • Help in developing clinical pathways, using evidence-based medicine and best practices
  • Potentially overseeing assigned nurse practitioners
  • Facilitating staff, patient and family decision-making by providing education and guidance





  • Licensed physician with an active Florida State License
  • Board certification
  • Experience in geriatrics, internal medicine or palliative care
  • Prior experience in the home or a community based setting preferred
  • Ability to work independently and manage time effectively
  • Strong interpersonal skills
  • Experience with an electronic medical record
  • Solid computer skills; Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records
  • A valid driver’s license and access to an automobile for travel
