Case Manager (RN) PRN


Responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical, financial, and psycho-social approach through the continuum of care. Performs concurrent clinical review to determine appropriateness of admission and continued need for care delivery in the acute care setting. Performs telephonic clinical reviews with claims insurance personnel to facilitate reimbursement for the delivery of patient care services. Assesses the patient for discharge planning needs and referrals to the interdisciplinary team. Supports collaboration and communication among the patient/family and entire health care team regarding the patient's plan of care and continued needs post-discharge and facilitates those services required. Discharge planning will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization. Identifies quality and/or risk issues and reports these to the appropriate individual/department.

Compact RN license or Colorado RN license accepted upon hire, CO license required within 90 days of hire.

Solid clinical knowledge base in medical/surgical nursing. Ability to assess information and apply critical thinking skills to determine course of action and relay this information to the interdisciplinary team.
