- Under minimal supervision, the Staff Nurse/Diabetes Nurse Educator provides services on an inpatient /outpatient and community basis at Overland Park Regional. Individual must be self-motivated and take initiative to be proactive in navigation of patient education needs. Actively involved in providing staff/community education. Responsibilities include but not limited to, assessment of the client's current knowledge and skill level of self-diabetes management; development of an individual plan of care; assisting the client to set achievable educational and behavior change goals; provides case management of diabetes disease process; monitors lab results and quality of life survey variances; (calculates initial insulin dosage, adjusts insulin and oral agent doses to achieve glycemic control per collaboration with physician on patients diabetes medication management.. Communicates with referring and/or primary care physician at scheduled intervals; documents client progress. Must learn and or work to maintain familiarity with the National Standards for Diabetes Self-Management Education and Support.

Requires graduate of school of nursing with BSN degree. Licensure/Certification:
Current licensure in the State of Kansas required. BCLS certification required. Certification as Diabetes Educator preferred. Must be eligible for CDE exam within 2 years of hire date or at first eligible exam with successful passing of exam within two attempts. Valid driver's license. Work Schedule:
Requires flexible hours to accommodate patient and community needs for evening classes and or weekend events. Experience/Skills:
2 years of clinical nursing experience with a focus on patient population with chronic disease; diabetes preferred. Computer skills preferred (word processing, Windows, Access). Experience with patient documentation and electronic medical records. Excellent interpersonal and problem-solving skills. Ability to case manage multiple client caseload.
