BAYADA Home Health Care has a full time immediate opening for a Registered Nurse, Transitional Care Manager working in the Camden County area. The Transitional Care Manager is responsible for identifying current and future clients and for facilitating the smooth transition of clients from facility to home. As Transitional Care Manager, you will focus on building and maintaining relationships with clients and families, discharge planners, social workers and other facility personnel. In this role you will utilize your nursing abilities to assess patients' medical and social needs to identify appropriate home care services. Working with the staff of local BAYADA Home Health Care office, you will lead the intake process and coordination of discharge care for clients that will be returning home. Travel throughout the county will be essential and required. Hours are Monday through Friday (8:30am to 5:00pm), and two weekends per month.
This position requires prior home care, case management or discharge planning experience. Excellent planning, organization and presentation skills are critical.
