Portsmouth Regional is a 209-bed acute care hospital located in Portsmouth, NH, and is part of HCA, the nation's largest provider of healthcare services. Responsible for the comprehensive occupational therapy care of assigned patients to include evaluation, program planning, treatment implementation, documentation and discharge planning.

POSITION RESPONSIBLITIES:

STANDARD I

Maintains confidentiality of patient and employee information in verbal, written and electronic information.
Supports hospital mission statement, philosophy & goals.
Adheres to hospital/department dress code including wearing unobstructed name badge at all times.
Records time worked and adheres to meal/rest breaks according to department policy/schedule.
Follows safety & security policies of hospital and is knowledgeable of emergency procedures.
Absence/Tardy occurrences are within policy guidelines: over 6 occurrences are unsatisfactory and subject to disciplinary action.
Assures patient awareness of patients' rights.

STANDARD V Position Specific Performance Criteria/Essential Job Functions
Identifies complex socioeconomic and psychosocial behaviors and refers appropriately for special intervention.
Identifies and documents precautions and implications for treatment intervention from medical history screening process.
Identifies and documents learning preferences and educational barriers. Utilizes the initial and discharge functional assessment indexes for goal and treatment planning.
Sets and documents realistic goals with time frames incorporating the patient and family needs and goals.
Maintains consistently high standard of complete and legible documentation per departmental policy and procedure.
Maintains statistics and reports accurately and in a timely manner.
Provides accurate and timely billing information on a daily basis.
Complies with UR policies and procedures - timely, accurate and complete information.
Provides accurate documentation and timely reporting for Medicare/Medicaid.
Incurs less than five instances of inability to obtain proper pre-approvals or authorizations for evaluation and treatment of MCO patients.
Performs specific tests and measures to identify patient problems to establish a therapy diagnosis.
Formulates treatment plan with measurable goals which address identified problems.
Implements all treatment techniques, modalities and equipment in accordance with the treatment protocols, policies, procedures and competencies of the department.
Assesses and documents patient response to treatment and learning on routine basis.
Assesses and documents patient discharge status, goal status and discharge needs/plan.
Evidence of appropriate written, dated, and signed home program and instructions with a copy in the chart.
Establishes a collaborative relationship with referring practitioners through effective communication and demonstrated referral patterns.
Effectively manages patient compliance, no shows, cancellations and adherence to program.
Demonstrates ability to problem-solve and utilize good judgment skills resulting in effective decisions made in a timely manner.

EXPERIENCE: (Minimum relevant work experience for successful job performance)
 Less than 1 year

EDUCATION:
 4 year/Bachelor's Degree

SPECIAL QUALIFICATIONS:
 BLS
