Support Patient Care Navigator (CCHW) - Emergency Room (ER).

Since 1973, Houston Northwest Medical Center has been providing comprehensive healthcare to the North Houston communities. The hospital is known for its maternity and women's services, including a Level II Neonatal ICU, and for its cardiac care as well as bariatric and diabetes programs. In addition to its hospital emergency room, Houston Northwest Medical Center operates two urgent care centers

Houston Northwest Medical Center is currently accepting applications for Support Patient Care Navigator (CCHW) - Emergency Room (ER).

RESPONSIBILITIES:
  • Effectively work with people (staff, clients, doctors, agencies, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers between patients and institutions.
  • Work collaboratively and effectively within a team.
  • Build and maintain positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff.
  • Facilitate communication and coordinate services between providers
  • Document activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place.
  • Provide information about Health issues and program services to the user population.
  • Convey the purposes and services of a program to the user population and the impact that program or service would have on that population.
  • Establish positive, supportive relationships with participants and provide feedback.
  • Travel to service locations as assigned.
  • Maintain confidentiality per HIPAA guidelines in regards to patient information.
  • Continuously expand knowledge and understanding of community resources, services and programs provided; human relations and the procedures used in dealing with the public as part of a service or program; volunteer resources and the practices associated with using volunteers, operations, functions, policies and procedures associated with the department or program area, procedures and resources available to handle new, unusual or different situations
  • Identify and apply appropriate role definition and skilled boundaries
Responsibilities to Clients/Patients:
  • Motivate patients to be active, engaged participants in their health.
  • Facilitate the understanding of health management/care plans and instructions.
  • Assist patient in accessing health related services, including but not limited to:
    • scheduling appointments
    • assisting with applications for programs for which they may be eligible
    • obtaining a medical home
    • scheduling appointments
    • assisting with applications for programs for which they may be eligible
    • obtaining a medical home
  • Providing instruction on appropriate use of the medical home:
    • overcoming barriers to obtaining needed medical care and social services
  • Any other related duties as assigned by supervisor/manager.

Qualifications:
QUALIFICATIONS:
  • Certification as a Community Health Worker (CCHW) required
  • 2-years' experience working in a Community or healthcare setting preferred
  • Knows and understands specific Health issues
  • Advanced computer skills
  • Bi-lingual preferred
  • Must have ability to perform complex and diverse administrative duties that involve application of procedures, independent analysis, and interpretation of data and demonstrate appropriate judgment.
