Under general direction of the Senior Nurse Manager or Nurse Manager, the remote Telehealth Nurse Educator (Patient Single Point of Contact) is responsible for serving as the patient and/or caregiver single point of contact for all interactions including clinical and reimbursement. This position will serve as the dedicated voice to the patient for product specific education, care coordination, and individualized adherence support outreach. In addition, the position will support on-going adherence by working with the patient on short and long term goals using care planning tools. Serving as the dedicated voice to the patient and/or caregiver, this position will also communicate the results of the benefits investigation, co-pays, patient assistance, and clinical trial, collaborate on shipment of free goods, conduct missing information outreach to patients and provide on-going commercial service support for the patient.

  1. Provide consumer support in a call center environment for patients receiving treatment.
  2. Risk assess patient for compliance and adherence to treatment regimen
  3. Identify barriers to care
  4. Support patient care planning and development of short and long term goals
  5. Provide information about disease process
  6. Provide emotional support to patient and family members
  7. Accepts inbound calls and makes outbound calls to enrolled patients
  8. Receives notification through, but not limited to: web portal, live chat, patient assistance program, fax enrollment form, electronic enrollment form or phone; calls patient to provide information about the product, program, send out information kits if needed and management advise for any side effects
  9. Enter all consumer activities accurately in clinical data base
  10. Intakes and reports adverse events as directed
  11. Provide case management support
  12. Provide patient support for payer policies and coverage issues
  13. Communicate reimbursement and financial assistance information
  14. Conduct missing information outreach
  15. Communicate completed benefits investigations
  16. Identify, enroll, and administer the co-pay program
  17. Collaborate with internal and external stakeholders to order, coordinate, and confirm shipments
  18. Must be able to work a flexible schedule and have flexibility for changing program needs
  19. Performs related duties as assigned

  1. Must possess a current license issued by a state or states or jurisdiction(s) within the United States, and be unrestricted in any state with the ability to obtain licensure in all 50 states
  2. Graduation from an accredited nursing program
  3. Experience with and/or potential to provide high quality customer service
  4. 3 years nursing experience required
  5. Duchenne Muscular Dystrophy required
  6. Neurology and pediatric experience preferred
  7. Telephonic experience a plus
  8. Certification may be required
  9. Graduation from an accredited nursing program
  10. Case management experience preferred
