Position Summary:
The Urology Clinical Nurse Coordinator provides leadership through the collaboration of the Director/Designee through planning, organizing, coordinating, monitoring and evaluating the care provided for urology patients. The delivery of care is based on criteria that is age appropriate and is based on the physical, psychosocial, cultural, safety and learning needs of the individual patients. Responsibilities include, but are not limited to: functioning as a member of the multidisciplinary team, serving as an advocate and educator for patients. The Urology Clinical Nurse Coordinator assists patients from point of entry, through diagnostic studies to diagnosis, and throughout the treatment care plan. The primary function is to build a relationship with patients and physicians, to coordinate a plan of care including appointments, education, provision and /or enablement of support services and representation within the multidisciplinary care environment. The coordinator also assumes responsibility and accountability for the management of resources to achieve efficient, high quality outcomes for each patient including support for interdisciplinary and cross facility collaboration e.g. case conferences, and referrer communication. On an aggregate level, the coordinator will track performance of the program in line with HCA, division and facility goals (growth, quality, practice guidelines, etc.) and identify opportunities to streamline care practice, for example, through development of evidence-based guidelines. The Urology Clinical Nurse Coordinator will serve as a liaison between the patient and family, primary care physician, internal and external care providers, specialists, referrers and support network members (e.g. social workers, and the wider healthcare community). This role will include conducting internal and external outreach and marketing. The Urology Clinical Nurse Coordinator will perform delegated duties consistent with the scope of practice and functions within the philosophy and overall plan of care at Swedish Medical Center and will display positive support for the overall goals, values and mission of Swedish Medical Center.
Qualifications:
Licensure/Certification/Registration:
Current Colorado Registered Nurse License.
Current BCLS

Education:
Bachelor Science Nursing or actively pursuing with completion within 3 years of hire required. Graduate of an accredited school of nursing required.

Experience:
Minimum of 3 years clinical RN experience required. One year of progressive supervisory, charge nurse, or coordinator experience preferred. Ability to articulate and demonstrate knowledge of nursing theory and practice required. Experience in program development, urology patient care, patient education, and/or leadership preferred.

Knowledge/Skills/Abilities:
Demonstrates the ability to have direct patient care and leadership responsibilities. Ability to recognize the needs and concerns of people and to maintain constructive relationships in all interactions. Adheres to all departmental safety initiatives. Ability to communicate effectively in oral and written manners. Ability to assume leadership role with the health team in planning and delivery of patient care.

2. Strong planning, analytical and written skills.
3. Ability to utilize and coach others in the nursing process.
4. Ability to utilize database, spreadsheet, word processing to conduct analysis of statistical reports.
5. Ability to educate, coach and mentor team using adult learning principles.

Degree of Supervision Required:
Minimal. Individual proceeds on own initiative, in compliance with the philosophy, policies, procedures and practices of SMC and in frequent collaboration with the Director/Designee.
